Direct Pricing cannot be combined with managed-care pricing, network hearing benefit pricing, vouchers, third-party administrator pricing, or other contracted discount programs. If your insurance plan allows reimbursement from an itemized invoice, Hear Florida can provide the documentation needed for claim submission. Insurance coverage, reimbursement, and patient responsibility are determined by the patient’s insurance plan. If you have already been referred, registered, authorized, or scheduled through a third-party administrator, managed-care hearing program, insurance hearing benefit, discount program, or network referral source, your purchase may be subject to that program’s contractual terms, program rules, guidelines, referral process, or opt-out requirements. Hear Florida may need to review your program option first, and a direct Hear Florida purchase may require additional documentation, program review, transfer of the referral, or opt-out paperwork before the purchase can be completed.
